When Does a Filling Need to Be Replaced?

As the saying goes, an ounce of prevention is worth a pound of cure. The earlier a problem is caught in regards to fillings that need replacing, the better. Having regular, scheduled checkups can help to avoid filling replacement from developing into a bigger, and possibly more expensive or more painful problem. The regular x-rays we take can discover issues that may not have been causing any discomfort to the patient, or visibility to the dentist. We do come across it in the office where a big problem could have been discovered much earlier and been much easier to take care of, for the patient too! What are Dental Sealants?

Molars are made up of canyons, caves, pits, and seemingly endless caverns that are a breeding ground for decay. One method to prevent this decay is applying dental sealants.

A sealant is made up of composite (a plastic-like) material that contains bonding agents to seal to the edge of the tooth. Sealants placed on the chewing surfaces of back teeth block food from being trapped. The process in which a sealant is placed is quite precise and painless.

Painless Application

First the tooth is cleaned with a sodium bicarbonate (baking soda) spray.

Then an acid etch is applied to “roughen up” the surface. No saliva is to touch the tooth. This will re-mineralize the area, then a repeat etching is needed. An alcohol-based liquid then dries out the area and it must remain completely dry. The sealant is placed and guided through all the caverns, pits, fissures, and grooves. It is then cured with a special light, which makes it a hard, plastic-like material.

How Long Do Dental Sealants Last?

Sealants can last for several years. It is wise to have them examined on a semi-annual basis. If there is a break in the sealant, a high risk for decay is common. If a sealant is damaged, repair is simple, painless, and quick to complete.

Who are Sealants for?

Anyone! Children often receive sealants as routine preventive care. Adults with deep canyons with stained grooves on their teeth can also benefit from a sealant. The process is quick, painless, and does not require any anesthesia. It is an effective way to lower dental restorative costs.

The Evolution of the Toothbrush

The evolution of the toothbrush, eco friendly toothbrush bristles

We have been thinking about toothbrushes and sustainability. Specifically, what were toothbrushes made of before they were plastic and if there are some good quality eco-friendly toothbrushes available now.

Early Toothbrushes

The earliest toothbrushes were found in China from the period of the Tang Dynasty (619–907) and adopted in Europe in the 17th century. The handles were usually bamboo or bone with hog hair bristles! Tooth brushes from these materials became mass produced and continued to be imported to England until the mid-20th century!

1600 BC – Chew Sticks

The earliest chew sticks were found from as far back as 1600 BC. These chew sticks were basically just a stick with a frayed end that would be used to clean the teeth.

1885 – Mass Production of Toothbrushes in the USA

In the United States, mass production of toothbrushes started in 1885. They were made with a bone handle and Siberian boar hair bristles. Wood or ivory was also used for the handle. Animal bristle was not considered ideal for the bristles because it may not dry out well and then could harbor bacteria.

1938 – Plastic Toothbrushes

The first nylon bristle toothbrush went on sale in 1938.

Eco-Friendly Options for Toothbrushes Now

The most common eco-friendly option around seems to be the bamboo handle with nylon bristles. Have you tried any yet? What do you think of them?

As of today, there aren’t any bamboo toothbrushes accepted by the American Dental Association or the Canadian Dental Association.

How to Take Care of Your New Dental Crown or Bridge at Home

Here are some basics on how to take care of you temporary or permanent crown or bridge including oral hygiene, what to do if your temporary crown is dislodged and some information around sensitivity.

Many, many people get crowns and bridges at our office. We know that if it is your first time with this type of restoration it can be a bit intimidating. Armed with the information here, we hope you will feel reassured.

What Are Temporary Crowns or Bridges?

A temporary is a crown or bridge that is placed on the prepared teeth while the final restoration is being made. The temporary protects the exposed tooth so it is not sensitive, prevents food and bacteria from collecting on the prepared teeth, and prevents the tooth from shifting or moving, which can make seating of the final restoration more difficult. The temporary is placed with lightweight cement that is designed to come off easily so avoid chewing sticky foods such as gum, caramels, etc.

Oral Hygiene and Your Temporary

It is very important to keep the tooth and surrounding gums very clean and healthy before your final appointment to cement the crown or bridge. If your gums are inflamed, bleeding will occur very easily during your appointment, making seating of the crown or bridge difficult. Follow the following home care instructions:
Use your toothbrush to clean the temporary as you normally do your other teeth.
When flossing, it is best to pull the floss through the contact rather than lift up on the temporary so you don’t accidentally loosen the temporary.
If your temporary comes off between appointments, slip it back on and call our office so that we can re-cement it for you. A little denture adhesive or toothpaste placed inside the crown can help to hold it in place in the interim.

Sensitivity

Sensitivity, especially to cold, is common for a time following treatment. It is normal to have discomfort in the gums around the tooth after the anesthesia wears off due to the procedure.
For the first few days, avoid extremely hot or cold foods and beverages.
If your gums are tender, rinse with warm salt water, dissolving ½ teaspoon of salt in an 8 oz. glass of warm water.
An analgesic such as Tylenol or Advil will help to increase your comfort.

Final Crown or Bridge

After the final cementation of your restoration, it may take a few days to get used to the new crown or bridge. If your bite feels unbalanced, or the new restoration feels high when you bite down, please be sure to call our office for an appointment for a simple adjustment. Other symptoms which show that your restoration needs adjustment is sensitivity to biting down or eating, and sensitivity to cold.

Home Care

Although crowns and bridges are often the most durable of all restorations, the underlying tooth is still vulnerable to decay, especially at the interface between the tooth and crown. It is important to resume regular brushing and flossing immediately. Daily home care and regulating your intake of sugar-containing foods will increase the longevity of your new restorations.

Tips For Dental Anxiety in Children

What to do if Your Child is Afraid of the Dentist

You’ve talked about going to the dentist, you’ve let your child know that dentist are important for healthy teeth and you’ve even explained how the visit might go. Yet, in the dentist’s chair, your child is not cooperating and is afraid.

We see many kids, from two months to teenagers who are afraid of the dentist. Here are some tips that can be helpful:

  1. Remain calm. Outwardly at least. If your child sees that you are upset, they will feed on that. Although remaining calm may not result in them immediately calming down, getting upset will almost certainly escalate their emotions.
  2. Accept that their feelings are valid. Even some adults are afraid of the dentist. It’s just that we are more able to rationalize ourselves through it.  All behaviour has a reason: your child might find the chair weird, the masks a bit scary and, even though modern dentistry involves very little pain and discomfort, the amount of discomfort they have experienced at the dentist may still seem too much.
  3. Suggest how they can cope. Remind them that discomfort will pass quickly and that they then can then relax and watch the TV if there is one, or check out the dentist’s artwork.  Calmly remind them that a little treatment now can prevent a big treatment later, and also that everyone goes to the dentist and it is what you are expecting of them as a member of a healthy family.
  4. Reward them afterwards. Remind them of the treats that come after a dental visit. Most kids love the little toy they get at the end of a visit, and some enjoy just receiving a new floss, mini toothpaste, or toothbrush.  Maybe you can do something nice for them after the visit. Do they like to pop into the coffee shop for steamed milk? Or maybe a new little toy or book or an activity, depending on what they like.
  5. Try visiting a dentist that caters to children. Maybe they really don’t like the particular dentist, or office or assistant they saw first. Often going to a dental office that sees many kids can be worthwhile. Staff are usually very patient, gentle, and have a lot of experience with various emotional situations for parents and kids. All this experience can go a long way in making the visit more pleasant.
  6. Try conscious sedation or general anesthetic. Conscious sedation may be good if your child is extremely afraid of the dentist but needs treatment in one area in one area of the mouth. Or, if the child is young and requires treatment in many ‘quadrants’ of the mouth (cavities in the top right, bottom right and bottom left), you may be able to take your child to dental surgery center where they are completely put under. A small child may not be able to tolerate fillings in more than three spots in one appointment.
